What helps the doctor confirm your diagnosis
- A letter, visit summary or chart note that names your diagnosis
- A list of the treatments and medications you have tried
- Recent test results or imaging, if your condition involves them

How to get your card
- Apply online and upload your driver's license or state ID.
- Talk with a doctor licensed in your state by video, often the same day.
- If you qualify, your doctor files your certification with your state's medical cannabis program. If your state charges a card fee, you pay it directly to the state.
- Your card arrives by mail, usually within 5 to 15 days.
